Indications & Dose:
Adult
Benign gastric and duodenal ulcers:
(including those complicating NSAID therapy), 20mg daily for 4 weeks in duodenal ulceration or 8 weeks in gastric ulceration; in severe or recurrent cases increase to 40mg daily; maintainance for recurrent duodenal ulcer, 20mg daily.
Duodenal ulcer associated with H. pylori:
40mg daily with amoxycillin 750mg twice daily for 2 weeks.
Zollinger-Ellison syndrome:
initially 60mg once daily, usual range 20-120mg daily (above 80mg in 2 divided doses).
Reflux oesophagitis:
20mg daily for 4 weeks, followed by a further 4-8 weeks if not fully healed; 40mg daily has been given for 8 weeks in reflux oesophagitis refractory to other treatment; may be continued at 20mg daily.
Acid related dyspepsia:
10-20mg daily for 2-4 weeks,according to response.
BY INJECTABLE
By intravenous injection:
over 5 minutes or by intravenous infusion: 40mg completely 1 hour before surgery. I|n other disease 40 mg once daily untill oral administration possible.
Children
Severe ulcerating reflex oesophagitis:
Children 10-20kg; 10mg once daily, increased if necessary to 20mg once daily for 4-12 weekes
Children over 20kg,; 20mg once daily, increased if necessary to 20mg once daily, increased if necessary to 40mg once daily for 4012 weekes, to be initiated by hospital paediatricians.
Note: Pediatric oral suspention not available
Contraindications:
In pregnancy and lactation.
Side Effects:
CNS: Asthenia, dizziness, headache GI: Abdominal pain, constipation, diarrhea, flatulence, nausea
MS: Back pain
RESP: Cough
SKIN: Rash
Cautions:
Precautions:
Not for maintenance therapy of duodenal ulcer or GERD; elderly; children; symptomatic response does not preclude gastric malignancy.
Old age: May be used.
Interaction:
Drugs
Carbamazepine, diazepam, digoxin, glipizide, glyhuride, nifedipine, nimadipine, nisoldipine, nitrendipine: Increased concentrations of these drugs
Cefpadaxime, cefuraxime, enaracin, ketoconazole: Decreased concentrations of these drugs
Glipizide, glyhrrnide, tolbutamide: Increased absorption of these drugs, potential of hypoglycemia
Methotrexate: Case report of elevated rnethotrexate concentration
Phenytoin: Increased phenytoin concentration
