Indications & Dose:
Neuromuscular blockade (intermediate duration) during surgery and intubation
Adult: 80–100 micrograms/kg; (by intravenous injection) maintenance 20–30 micrograms/kg, adjusted according to response, max. 100 micrograms/kg in caesarian section, alternatively (by intravenous infusion) maintenance 0.8–1.4 micrograms/kg/minute, adjusted according to response
DOSES AT EXTREMES OF BODY-WEIGHT
To avoid excessive dosage in obese patients, dose should be calculated on the basis of ideal bodyweight
Contraindications:
Hypersensitivity.
Side Effects:
Transient fall in blood pressure, slight increase in heart rate, reduction in GI motility, rarely bronchospasm and anaphylactoid reaction.
Cautions:
Reduce dose in renal, hepatic failureimpairment.
Precautions:
Dose adjustment may be required in infants. Severe obesity. Malignant hyperthermia.
Pregnancy: Safety not established.
Breast-feeding: Use only if clearly indicated.
Old age: Use with caution.
Interaction:
Drugs
Incompatibility with alkaline solution (barbiturates such as thiopentone sodium).
Inhalational anaesthetics: Halothane, isoflurane may enhance effect.
Antiarrhythmic: Lignocaine, quinidine and verapamil may enhanced effect.
Antibacterials: Aminoglycosides, lincosamide, polymixins and rarely tetracyclines may enhance effect.
Anticholinestrases: Neostigmine may enhance effect.
Antiepileptics: Carbamazepine, and pheny-toin may reduce effect.
Ganglion blockers: May enhance effect.
Xanthines: Aminophylline and theophylline may antagonise the effect.
