Indications & Dose:

Complete or partial reversal of narcotic depression, including respiratory depression; diagnosis of suspected acute opioid overdosage; reversal of alcoholic coma,* refractory shock, * Alzheimer’s type dementia, schizophrenia,* anaphylaxis*


Adult


• Narcotic overdose (known or suspected):


SC/IM/1V 0.4 2 mg initially, repeat at 2 3 min intervals up to 10 mg, if no response after 10 mg reevaluate diagnosis


• Postoperative narcotic depression (partial reversal):


IV 0. I  0.2 mg at 2--3 min intervals to desired level of reversal (adequate ventilation and alertness without significant pain or discomfort); repeat doses may be required within 1 2 hr intervals


Child


• Narcotic overdose (known of suspected):


SC/1M/IV 0.01 mg/kg initially, give subsequent doses of 0.01 mg/kg prn at 2 3 min intervals


• Postoperative narcotic depression (partial reversal):


IV 0.005 0.01 mg q2 3 min to desired degree of reversal


Neonates:


BY SUBCUTANEOUS, INTRAMUSCULAR, OR INTRAVENOUS INJECTION


10micrograms/kg, repeated every 2 to 3 minutes or 200 micrograms  (60 micrograms / kg)  by intramuscular injection as a single dose at birth (onset of action slower)

CNS: Seizures, tremulousness


CV: Hypertension, hypotension, tachycardia, ventricular dysrhythmia


GI: Nausea, vomiting


RESP: Pulmonary edema


SKIN: Sweating


MISC: Reversal of analgesia
