Indications & Dose:

Vancomycin-resistant Enterococcus faecium infections including bacteremia, nosocomial or community acquired pneumonia, complicated skin and skin structure infections caused susceptible organisms  
DOSAGE

Adult

Vancomycin-resistant Enterococcus faecium infections including bacteremia: PO or IV 600 mg q12h for 14-28 days

Nosocomial pneumonia: PO or IV 600 mg q12h for 10-14 days

Community-acquired  pneumonia: PO or IV 600 mg q12h for 10-14 days

Complicated skin and skin structure infections:

PO or IV 600 mg q 12h for 10-14 days

Uncomplicated skin and skin structure infections: PO 400 mg q12h for 10-14 days

Child

Community-acquired pneumonia, nosocomial pneumonia, skin and skin structure infections, vancomycin-resistant Enterococcus faecium infections including bacteremia: PO or IV 10 mg/kg q12h for 14-28 days. 

Contraindications:

Side Effects:

CNS: Dizziness, headache (7%), insomnia EENT’ Altered taste, thrush

Gl: Constipation, diarrhea (8%), increased transaminase levels (7%), nausea, pseudomembranous colitis, vomiting

GU: Vaginal candidiasis

HEME: Agranulocytosis, anemia (7%), aplastic anemia, leukopenia (2%), thrombocytopenia (5%)

SKIN: Rash

Monoamine oxidase inhibition, uncontrolled hypertension, pheochromocytoma, carcinoid syndrome, or untreated hyperthyroidism, myelosuppression (including anemia, leukopenia, pancytopenia, and thrombocytopenia)

Drugs

Amphetamines, alcoholic beverages containing tyramine, metaraminol, phenylephrine, phenylpropanolamine, pseudoephedrine, tyramine: Severe hypertensive reaction

Antidepressants, cyclic: Excessive sympathetic response

 Dextromethorphan: Severe hypertensive reaction

Dopamine: Severe hypertensive: reaction :

Epinephrine: Severe hypertensive reaction

Levodopa: Hypertension

Meperidine: Severe hypertensive reaction Selective serotonin reuptake inhibitors (SSRls):

Serotonin syndrome (hyperpyrexia, cognitive dysfunction)

Sibutramine: Serotonin syndrome (hyperpyrexia, cognitive dysfunction)

Trazodone: Serotonin syndrome I (hyperpyrexia, cognitive dysfunction)

Venlafaxine: Serotonin syndrome (hyperpyrexia, cognitive dysfunction)
