Indications & Dose:


Adult


• Herpes zoster:


250mg 3 times daily for 7 days  or 750mg once daily for 7 days ( immunocompromised 500mg three times daily for 10 days)


• Genital herpes simplex:


250mg 3 times daily for 5 days; if CrCI 20-39 ml/ min, use 125 mg q24h for 5 days; if CrCI <20 ml/ min, use 125 mg q48h for 5 days


• Suppression of genital herpes:


250 mg bid; if CrCI 20-39 ml/min, use 125 mg q 12h; if CrCI <20 ml/min, use 125 mg q24h


Child:Not recommended
