Desferoxamine Mesylate

Indications & Dose:

Adult



IM 1 g stat, then 0.5 g q4h for 2 doses, additional doses of 0.5 gq4-l 2h prn, max 6 g/day; IV infuse at a rate < l5 mg/kg/hr, dosage the same as TM max 6 g/day (use only in cardiovascular collapse)


Chronic iron overload: 



TM 0.5-I g/day; SC via portable pump INF 1-2 g/ day over 8-12 hr; IV give 2 g with, but separate from, each unit of blood at a rate < 15 mg/kg/hr


Child


•  Acute iron intoxication:



IM 90 mg/kg q8h, max 6 g/day; IV 15 mg/kg/hr, max 6 g/day 


Chronic iron overload:



IV 15 mg/kg/hr, max 6 g/day; SC via portable pump INF 20-40 mg/kg/day over 8-12 hr 

Contraindications:

Severe renal disease or anuria 

Side Effects:

CV: Hypotension (with rapid IV inj), tachycardia
GI: Abdominal cramps, diarrhea
EENT: Blurred vision, cataracts, ototoxicity
MS: Leg cramps
SKIN: Erythema, pain at inj site, proritus, urticaria
