Indications & Dose:

Psychotic disorders, short-term treatment of non-psychotic anxiety (not druf choice for most patients) Peptic ulcer and gastro intestinal disorders complicated by nervous & emotional factors Anxiety: Antiemetic:

• BY MOUTH

Adult: 2-4mg daily in divided doses, maximum 6mg

Elderly: Initially 1mg, adjust according to response

Children: Under 3 years not recommended- 3-5 years, up to 1mg daily, 6-12 years, up to 4 mg daily all in divided doses.

Contraindications:

Severe toxic CNS depression, coma, subcortical brain damage, bone marrow depression.

Side Effects:

CNS: Agitation, anxiety, confusion, depression, drowsiness, euphoria, exacerbation of psychotic symptoms including hallucinations, catatonic¬ like behavioral states, extrapyramidal symptoms (pseudoparkinsonism, akathisia, dystonia), headache, insomnia, lethargy, neuroleptic malignant syndrome, restlessness, seizures, tardive dyskinesia

CV: ECG changes, hypertension, hypotension, tachycardia

Eye/ENT: Blurred vision, cataracts, dry eyes, glaucoma, pigmentation of retina or cornea, retinopathy, vertigo

GI: Anorexia, constipation, diarrhea, dry mouth, dyspepsia, hypersalivation, nausea, vomiting

GU: Gynecomastia, impotence, increased libido, menstrual irregularities, priapism, urinary retention

HEME: Agranulocytosis, anemia, aplastic anemia, hemolytic anemia, leukocytosis, minimal decreases in red blood cell counts, transient leukopenia

METAB: Breast engorgement, hyperglycemia, hypoglycemia, hyponatremia, lactation, mastalgia

RESP: Bronchospasm, increased depth of respiration, laryngospasm

SKIN: Diaphoresis, loss of hair. maculopapular and acneiform skin reactions, photosensitivity

MISC: Heat or cold intolerance

 Elderly, children < 12 yr, prolonged use, severe cardiovascular disorders, epilepsy, hepatic or renal disease, glaucoma, prostatic hypertrophy, severe asthma, emphysema, hypocalcemia (increased susceptibility to dystonic reactions)

Interaction:

 Drugs

Anticholinergics: Inhibited therapeutic response to antipsychotic: enhanced anticholinergic side effects

Antidepressants.: Increased serum concentrations of some cyclic antidepressants

Barbiturates: Reduced effect of antipsychotic

Bromocriptine, lithium: Reduced effects of both drugs

Guanethidine: Inhibited antihypertensive response to guanethidine

Levodopa: Inhibited effect of levodopa on Parkinson’s disease

Narcotic analgesics: Excessive CNS depression, hypotension, respiratory depression

Orphenadrine: Reduced serum neuroleptic concentrations; excessive anticholinergic effects
